★　You must enrol in the National Health Insurance plan (Kokuminkenkouhoken) if;
　　you are to stay in Japan for longer than one year; and you have completed alien
　　registration procedures; and you are unable to enrol in other health insurance
　　programmes. Once enrolled you will be issued with a National Health Insurance card.
★　In the event that you move to another city, or you enrol in your employer's health
　　insurance programme, you must report this change to City Hall and return your
　　National Health Insurance card.
